After the questionnaire is received, we will send you a follow up email if we need more information. If your appointment request is not accepted and you did not receive an email from us a week before your appointment, please reach out to us to ensure that your submission was received.

Be sure and answer the questions as detailed and honest as possible. Your appointment will not be accepted until this process is complete. If not completed 1 week before your appointment date, your appointment request will be denied allowing other clients to book at that time.
